Fort Recovery’s Veterans Plaza – Name Nomination Form

Have you ever wondered how it is you add a service member’s name to the wall at the Fort Recovery’s Veterans Plaza? Well, your wait is over!

The Fort Recovery Veterans Committee has recently created a formal nomination form to be used to add additional names to the already hundreds of names that currently decorate the wall at the Fort Recovery Veterans Plaza.

For a name to be eligible for recognition, all of the following criteria must be satisfied:

  • The nominee is a living or deceased veteran, or is currently serving in the United States Military;
  • The nominee must be honorably discharged or is currently in good standing if currently serving; and
  • The nominee must have either lived or currently lives in the Fort Recovery School District, is a living or deceased member of the Fort Recovery Legion Post 345 or is a living or deceased member of the VFW Post 6515.

If you would like to nominate a service member for recognition on the wall at the Fort Recovery Veterans Plaza, click on the nomination form below and complete the form. All forms should be mailed, emailed, or hand-delivered to:
